REPORT OF UNSAFE CONDITIONS
- Any employee wishing to report an unsafe or unhealthy working condition or wishing to make a recommendation pertaining to safety or improving the health of the work environment may do so by completing the Report of Unsafe Condition Form (See Policy 8.1, Safety Programs, Inspections and Fire Evacuation Plans, Addendum C). All recommendations will be reviewed by the facility Safety Committee.
- .The person submitting the safety recommendation will be advised as to the action taken to remedy the condition. The employee’s right to file this report is protected by State law.
